Description

This is an annual reduced fee dental plan that allows individuals and families to receive quality dental care from The Dental Health Center for a percentage off the office’s usual, customary and reasonable (UCR) fees. This is not an insurance plan, no claims are filed, and no payments are made to any other healthcare providers. The Dental Health Center is not a licensed insurer, health maintenance organization, or other underwriter of health services. This plan may not be combined with any other offers, discounts, insurance, discount plans or advertisements.

Membership

Once the annual fee is paid, you will be entitled to the dental benefit membership program for 12 consecutive months. You have the right to cancel at any time, by submitting a written request however, this plan is non-refundable.

The Membership Includes:

(2) standard cleanings (exludes deep cleaning/scaling and root planing)
(1) Set of digital xrays: 4 Bitewing x-rays (cavity detection x-rays) and 2 periapical x-rays (PA)
(2) Exams per year
(2) Fluoride treatments
plus-15% Discount on all dental services
(discount rate INCLUDES!: 3-D cone beam imaging, Cosmetic Services AND Implants)

The above services without this plan would cost approximately $500!

Low Annual Fee- $325 PER PATIENT. $275 for each additional family member

The Dental Health Center is a full-service dental practice, combining general and cosmetic dentistry with family (ages 3 and up!) and implant dentistry.

Limitations and Guidelines

This plan is non-refundable. No refunds or premiums will be issued at any time if participant decides not to utilize their dental membership. You will not receive a membership card – your plans effective date will be on file with our office.

THIS PROGRAM IS AN IN-OFFICE DISCOUNT PLAN, NOT A DENTAL INSURANCE PLAN. THIS PROGRAM CANNOT BE USED:

In conjunction with another dental plan or offer – For services covered under worker compensation – For treatment which, In sole opinion of the treating dentist, Lies outside the realm of their capability – For referrals to specialists – For hospitalization or hospital charges – For cost of dental care which is covered under auto medical.

This plan is only covered at the office of The Dental Health Center. It can not be used at any other office. THIS PLAN IS NON-REFUNDABLE. No refunds or premiums will be issued at any time if participant decides not to utilize dental plan. The 12 month membership is due in full upon joining. Membership is effective on the day on which payment is received. Payment is due when services are rendered. It is the sole responsibility of the member to maximize benefits by arranging the appropriate appointments within the 12 months membership period. If the appointments are not used, the member will not be entitled to a refund. Renewal payment is due the beginning of the same month of original joining each year.
