| Your initial appointment will consist of a comprehensive initial exam and x-rays (if none have been provided) explaining your diagnosis and treatment options. Occasionally, treatment can be performed the same day as the consultation.
Please assist us by providing the following information at the time of your consultation:
• Previous X-rays (if applicable)
• A list of your current medical prescriptions
• If you have medical or dental insurance, bring your dental card. This will save time and allow us to help you process any claims.
• If X-rays have previously been taken by a dentist, you may request that they forward them to our office (click to download our X-Ray release form). If there is not enough time, please pick them up and bring them to our office at your appointment. If additional films are necessary, we can take them at our facility.
